RESUMO

PURPOSE: To determine the absorbance coefficient of the thin porcine cornea to ultraviolet-A radiation (365 nm) submitted for crosslinking. METHODS: This in vitro, benchtop experiment using cadaver tissue study analyzed 12 porcine corneal lamellas, which were obtained using a microkeratome after mechanical de-epithelization and separated into three thickness groups: 180, 300, and 360 µm. The corneal thickness values were measured by anterior-segment optical coherence tomography. All lamellas had ultraviolet-A (365 nm) absorbance measured with a 96-well plate spectrophotometer using an ultraviolet transparent microplate before riboflavin instillation and preand post-crosslinking according to the Dresden protocol. RESULTS: The ultraviolet absorbance profiles of the 180, 300, and 360 µm groups were obtained as α-coefficients of 12.85, 76.55, and 120.27, respectively. A theoretical formula was calculated though a statistical analysis that demonstrated the correlation between stromal lamellar thickness and ultraviolet absorbance. CONCLUSIONS: Corneal thickness and ultraviolet-A spectral absorbance of corneal lamellas showed linear correlation. These findings can potentially contribute to the optimization of ultraviolet-A application during crosslinking, making the treatment of corneas with thickness <400 µm safe and personalized energy delivery for each corneal thickness.

RESUMO

INTRODUCTION: Radiological indicators in head computed tomography (CT) scan have emerged as tools to evaluate sarcopenia using the sectional area and thickness of the temporal muscle. They can be obtained by standardized measurements in preoperative image assessment of patients with brain aneurysms. We aimed to evaluate the association between functional outcomes after interventions for intracranial aneurysms (IAs) and temporal muscle thickness (TMT) and area (TMA), as surrogates of sarcopenia. METHODS: This is a prospective observational cohort study in patients who underwent microsurgery or embolization for ruptured or unruptured IA between January 2018 and December 2019, with a 6-month follow-up. Preoperative CT scans were analyzed to measure TMT and TMA. The functional outcome was assessed by the modified Rankin Scale (mRS). The main outcome was the relationship between sarcopenia and the postoperative functional outcome. RESULTS: A total of 361 patients were included, of whom 199 (55.1%) had ruptured and 162 (44.9%) had unruptured lesions. Larger TMA significantly predicted better functional outcomes at discharge. TMT was associated with functional outcomes at both discharge and 6 months, adjusted for rupture and hypertension. Maximizing the sum sensitivity-specificity, an optimal TMT cutoff of 6.25 mm can predict unfavorable outcomes. Maximizing the positive predictive value × negative predictive value of a product, the cutpoint was 3.55 mm. CONCLUSIONS: Sarcopenia, represented by TMT and TMA, is associated with poorer functional results at discharge and 6-month follow-up in IA surgery. TMT below 6.25 mm was associated with unfavorable functional outcomes. These easily obtainable measurements may improve the decision-making process for patients with IAs.

RESUMO

OBJECTIVE: The objective of this study is to evaluate protamine sulfate effects on graft's blood flow by comparing transit-time flow measurement (TTFM) values before and after protamine administration. METHODS: This is an observational study with data collected between years 2018 and 2020. Immediate graft patency was evaluated using TTFM. Only patients with TTFM parameters registered before and after protamine infusion were included. The main three parameters studied were:  mean graft flow  (MGF),  pulsatility index (PI), and  diastolic flow (DF). In the first analysis, all conduits were evaluated regardless of the surgical technique used. In a second analysis, on-pump and off-pump groups were compared. Evaluated grafts were left internal thoracic artery, saphenous vein graft (SVG), radial artery, and right internal thoracic artery. Since SVG was numerically the most used graft, an exclusive analysis was created. RESULTS: Our study included 575 patients, resulting in a total of 1686 grafts, mean 2.93 grafts/patient. Off-pump surgery was performed in 158 patients. Before protamine infusion, inadequate TTFM parameters were observed in 3.8% of grafts. Overall, after protamine administration, MGF decreased in all grafts, but its reduction was not statistically significant. PI values increased in the SVG and DF values reduced in LIMA grafts. SVG group analysis showed that after protamine PI values were higher in OM1 and RCA. DF values increased in RCA. The comparison between off and on-pump surgeries, showed that in off-pump cases TTFM measures did not present statistically significant differences. CONCLUSION: Significant variations were observed in TTFM values before and after protamine administration. Although different, those values remained within the normal reference ranges. We recommend that flow measurement should be performed before protamine infusion.

RESUMO

Craniopharyngiomas (CPs) are benign tumors that are believed to arise from embryonic remnants of the Rathke pouch epithelium. Herein, we report a case of aseptic meningitis due to spontaneous rupture of multicystic CP, which contained an Ommaya catheter. A 19-year-old boy was admitted to the hospital with a 4-day history of acute severe headache after strenuous physical exercise followed by altered sensorium, fever, and neck stiffness. Cerebrospinal fluid (CSF) analysis revealed marked pleocytosis and elevated protein levels. CSF culture was otherwise negative. Cyst reduction on subsequent imaging confirmed the diagnosis. The patient received intravenous steroid therapy and was discharged asymptomatic. This is a rare evolution of a multicystic CP, which was previously treated with intracystic therapy and had an Ommaya catheter. Clinicians should be aware of spontaneous CP rupture and look actively for the occurrence of cholesterol crystals or elevated CSF levels of cholesterol as well as prompt follow-up imaging.

RESUMO

Lutembacher syndrome is characterized by the association of mitral stenosis and atrial septal defect (ASD), usually of the ostium secundum type. The association between superior vena cava-type ASD and partial anomalous pulmonary venous connection is unusual and there are few descriptions in the literature. We report this condition in a 24-year-old woman who was admitted to the hospital with a 1-year history of progressive dyspnea and describe the successful surgical repair following mitral commissurotomy and tunneling of the anomalous pulmonary veins to the left atrium, which determines the closure of the ASD and tricuspid repair.

RESUMO

There are four types of anastomoses between themedian and ulnar nerves in the upper limbs. It consists of crossings of axons that produce changes in the innervation of the upper limbs, mainly in the intrinsic muscles of the hand. The forearm has two anatomical changes Martin-Gruber: branch originating close to the median nerve joining distally to the ulnar nerve; and Marinacci: branch originating close to the ulnar nerve and distally joining the median nerve. The hand also has two types of anastomoses, which are more common, and sometimes considered a normal anatomical pattern Berrettini: Connection between the common digital nerves of the ulnar and median nerves; and Riche-Cannieu: anastomosis between the recurrent branch of the median nerve and the deep branch of the ulnar nerve. Due to these connection patterns, musculoskeletal disorders and neuropathies can be misinterpreted, and nerve injuries during surgery may occur, without the knowledge of these anastomoses. Therefore, knowledge of them is essential for the clinical practice. The purpose of the present review is to provide important information about each type of anastomosis of the median and ulnar nerves in the forearm and hand.

RESUMO

OBJECTIVE: To investigate the role of ICRS in halting keratoconus progression in a large sample of patients. METHODS: A retrospective, non-comparative, interventional study based on the review of medical records of patients diagnosed with keratoconus who underwent Ferrara corneal ring segment implantation. A retrospective chart analysis study of 123 operated eyes with follow-up ranging from 3 to 16 years (mean 5.3 ± 3.6 years) was performed. This study was carried out at Ocular Surgery Unit, São Paulo, Brazil. All topographic data were obtained from Pentacam (Oculus, Arlington, USA). The same surgeon performed all surgeries, and the Ferrara ICRS nomogram was used for ICRS selection in all cases. RESULTS: Corrected distance visual acuity, keratometry, and topographic astigmatism improved in most cases, with statistical significance. In 42 eyes (53.8%), there was an increase in K1 or K2, and in 36 (46.2%), there was a reduction or maintenance in K1. Considering a difference higher than 1 D, between 3-month post-surgery and final visit (group 3), 32 eyes (41%) showed an increase, and 46 eyes (59%) ended equal or below this value. CONCLUSION: The implantation of ICRS showed improvement in visual and keratometric indexes. The majority of patients did not increase more than one diopter in keratometric values after ICRS implantation. ICRS may be effective for slowing disease progression, especially in older patients.

RESUMO

Veterinary Ophthalmology provides complementary information for the diagnosis of ocular pathologies. Studies in wild species are essential. Among the diagnostic techniques in ophthalmology, two-dimensional ultrasonography stands out. The agouti is a rodent belonging to the Dasyproctidae family that has been widely used as an experimental model. For these animals, sight is one of the crucial senses for their survival. The aim of this study was to evaluate the effectiveness of the two-dimensional ocular ultrasound technique to obtain anatomical measurements and the external ophthalmic artery resistivity index, presumably normal in the species Dasyprocta prymnolopha. Forty eye bulbs of 20 adult rodents of the species were evaluated by ultrasonography. In these animals, B-mode echobiometry was performed using the transpalpebral approach and the hemodynamic study of the external ophthalmic artery using the color Doppler technique. All examinations were initiated by the left eye bulb and all measurements were performed by only one examiner. The collected data related to echobiometry were analyzed using Bioest 5.0 for Windows. Initially, normality was tested using the Shapiro-Wilk test for each parameter, then the paired t-test was performed, comparing right and left eyes, and a significance level of 5% (P < 0.05) was adopted. Based on the methodology used, the following values were obtained for the right and left eyeballs, respectively: anterior chamber thickness - mean of 1.28 ± 0.3 mm and 1.22 ± 0.1 mm; lens thickness - 8.27 ± 0.9 mm and 8.11 ± 0.9 mm; vitreous chamber thickness - 5.35 ± 0.48 mm and 5.30 ± 0.47 mm and axial length - 12.7 ± 0.9 mm and 13 ± 0.68 mm. The mean external ophthalmic artery resistivity values were 0.4305 ± 0.0390 and 0.4258 ± 0.0387 (right and left eye, respectively), characterizing a low resistance. There was no statistical difference between the right and left eyeballs in any of the studied parameters. The use of the convex transducer was feasible, promoting adequate contact with the ocular surface and images of satisfactory quality for obtaining measurements, similar to what was observed in studies evaluating the ocular biometry of primates and dogs. The anterior chamber thickness values in this experiment did not differ statistically between the antimers, as well as observed for dogs. The data obtained for lens thickness did not differ statistically for antimers, like those obtained for other rodent species evaluated with the same methodology. The mean values of vitreous chamber thickness were like those observed in chinchillas but correspond to about half of that obtained for capybaras. In this study, the external ophthalmic artery was characterized in all animals, but obtaining the spectral tracing was difficult due to its fine caliber. In wild animals, and especially in wild rodents, there are few data reporting the resistivity of the ophthalmic artery, and there is a lack of studies, which can be explained by the behavioral characteristics of defense and by the high susceptibility to stress in capture, since the performing the technique requires, as in other procedures, the use of chemical containment.

PURPOSE: To evaluate anatomical differences between laser (VICTUS) and manual constructed phacoemulsification incisions. SETTING: Study was conducted at the Hospital de Olhos Paulista, Sao Paulo, Brazil. DESIGN: This is a prospective, observational, consecutive nonrandomized cohort. PATIENTS AND METHODS: Sixteen patients (18 eyes) had phacoemulsification surgery, nine eyes had manual incisions and nine eyes had femtosecond incisions. All incisions were performed by the same surgeon. All incisions were evaluated at the 1st and 30th postoperative day with anterior segment optical coherence tomography to evaluate anatomical features. Inclusion criterion was presence of low visual acuity due to cataract. Exclusion criteria were other ocular pathologies, previous trauma, or corneal wound healing disease, as any intra or postoperative complication. Three different corneal specialists analyzed wound architecture features (epithelial or endothelial gap, incision edema, number of planes, Descemet's detachment, and loss of wound sealing) at both visits. ImageJ (National Institutes of Health) software was also used to analyze anatomical features of incisions. RESULTS: Almost all incisions had edema at the first postoperative day. All femto incisions were triplanar, as intended, unlike manual incisions, which showed two uniplanar incisions (P=0.009). All laser incisions presented partial loss of wound sealing, while five keratome incisions had no loss of wound sealing at the first postoperative day (P=0.03). CONCLUSION: These findings support the hypothesis that laser constructed incisions are more precisely constructed than manual, although corneal wound healing was similar between the two groups at the 30th postoperative day, with a tendency to a faster anatomical improvement in the keratome group.

RESUMO

Wild species, especially those threatened with extinction, are increasingly being investigated to obtain information that can be useful for their preservation. The objective of the present study was to standardize the vertebral heart scale (VHS) and cardiothoracic ratio (CTR) of the collared peccary (Tayassu tajacu Linnaeus, 1758) sedated with ketamine and midazolam. Fourteen clinically healthy collared peccaries were examined in the two-year age group weighing 15-22kg. The animals were submitted to digital radiography of the thorax in lateral and dorsal ventral projections to calculate the VHS and CTR. The VHS mean values for males and females was 8.88±0.51v for right recumbency and 8.84±0.39v for left decubitus, and there were no significant between-gender differences regarding recumbency (p>0.05). The CTR showed mean values of 0.50±0.05 (males) and 0.45±0.04 (females), but the gender-differences were not significant (p>0.05). A positive correlation was shown between VHS and CTR (r=0.98, right decubitus; r=0.96, left decubitus). Establishing reference values for heart measurements in collared peccaries using digital radiography of the thorax permitted standardization of the VHS and CTR values for this wild species. In the studied wild animal model, the VHS and CTR heart assessment indexes were shown to be essential diagnostic tools for investigations of alterations in the size of the cardiac silhouette.(AU)

Agoutis are small-sized wild animals whose body weight can reach up to 4kg, and are found throughout Brazil. They are considered important seed dispersers, especially for big trees and there are species that rely almost exclusively on these animals for their territorial distribution. The objective of the present study was B scan and Doppler ultrasound characterization of the abdominal organs of healthy agoutis reared in captivity. Fifteen agoutis, chemically restrained, were used from the Nucleus for Wild Animal Studies and Conservation (Núcleo de Estudos e Preservação de Animais Silvestres - NEPAS), CCA-UFPI, submitted to B scan and Doppler ultrasound examination. The urinary bladder wall was hyperechogenic, thin, smooth and regular throughout its anatomic path, with 0.09±0.03cm mean thickness. The kidneys showed fine and homogeneous echotexture, preserved global echogenicity, hyperechogenic in relation to the spleen and isoechogenic or discreetly hyperechogenic in relation to the liver. The spectral Doppler trace showed systolic and diastolic peaks, wide and thread-like, with low flow resistance and a continuous and full diastolic portion that decreased gradually during the diastole (75.83±1.42cm/s, for the right kidney and 80.43±1.22cm/s, for the left kidney). The right adrenal gland was 0.61-1.18cm long and 0.17-0.32cm in diameter, while the left adrenal gland was 0.62-1.16 long with 0.14-0.25cm diameter. The agouti spleen was filiform in shape, with pointed poles and 1.02±0.18cm in diameter. The agouti liver occupied all the abdominal cavity cranial space in direct contact with the diaphragm. The intrahepatic vascular flow allowed individualization of the portal vein (PV) and hepatic vein (HV). The portal veins were distinguished from the hepatic veins mainly by their wall echogenic pattern. The pancreas was 0.51±0.1 cm thick and the pancreatic duct measured 0.12±0.02cm. The stomach was placed to the left the spleen and to the right of the proximal intestine and the transversal colon and the walls were 0.16±0.05cm thick. The abdominal aorta was 0.43±0.04cm in diameter and showed 95.2±2.16cm/s vascular flow. This study characterized agouti organs and abdominal blood vessels by B scan and Doppler ultrasound, that permitted definition of the size, shape, position, echogenicity and echotexture of the anatomic constituents and established reference values for the vascular network and blood flow in the species.(AU)

Capuchin monkeys are a species of arboreal primate found in all South American countries. These monkeys have been highlighted for their potential for biomedical research due to their anatomic and physiologic similarities and genetic homology with humans. Here we characterized the electrocardiographic tracings from 12 healthy, young capuchin monkeys that were restrained with ketamine and midazolam. All 12 monkeys had normal sinus rhythms. Neither P-wave duration, PR interval, QT interval, nor P- or R-wave amplitude (in millivolts) differed between males and females. The P waves were small, monophasic, and positive in all animals. The QRS complex showed positive polarity in the D1, D2, aVL, aVF, V2, V4, and V10 derivations and negative polarity in the D3, aVR, and rV2 leads. The T wave exhibited a negative polarity only in the aVR derivation in all animals in the study, and no significant difference was present between sexes. The ST segment was isoelectric in both sexes and lacked reductions and elevations. The anesthetic protocol was well tolerated all of the monkeys and allowed for diagnostic-quality acquisition, measurement, and characterization of the electrocardiogram and establishment of the normal electrocardiographic parameters of chemically restrained capuchin monkeys.

Introduction: The capuchin monkey is a type of nonhuman primate that has shown great potential for preclinical studies because of its anatomical and physiological similarities to humans. Objective: To study the indices of myocardial deformation in anesthetized capuchin monkeys using speckle tracking. Methods: Sixteen animals from the Zoobotanical Park, Teresina, Piauí, Brazil, were used and chemically restrained using a combination of ketamine and midazolam. Echocardiography recordings were obtained in B, M and Doppler modes, and strain and strain rate were measured using speckle tracking. Results: The variables that showed statistically significant correlation coefficients in relation to weight were LVFWd, LVIDd, LVIDs, E wave, A'RV, MAM and TAPSE. HR showed a positive correlation with the E wave and A wave and a negative correlation with IVRT. FS presented a positive correlation with the E/A wave relationship (r = 0.61). TAPSE showed positive correlations with E'RV and A'RV. The values obtained for circumferential (-18.17 ± 4.68%), radial (47.13 ± 5.24%) and longitudinal (- 26.46 ± 5.15%) strain for the capuchin monkeys were within the normal ranges for males and females. Conclusion: The present study provides the first reference values for echocardiographic measurements in B, M and Doppler modes for capuchin monkeys anesthetized with ketamine and midazolam. The strain and strain rate values obtained using speckle tracking showed similarities with those obtained in humans, suggesting that this tool has the potential to be exploited in preclinical studies using the capuchin monkey model

RESUMO

B-scan ultrasonography is an important diagnostic tool that allows characterization of internal organ anatomy and, when complemented by Doppler ultrasound, allows vascular hemodynamic assessment, increasing the diagnostic accuracy. Thus, the aim of the present study was the B-scan ultrasound characterization and measurement of the eyeball segments and assessment of the external ophthalmic artery by color and pulsed Doppler. Sixty eyeballs were assessed from 30 dogs of different breeds using an 8.5MHz microconvex transductor. First, biometry was performed by B-scan of the following segments: axial length (M1), anterior chamber depth (M2), lens thickness (M3), lens length (M4), vitreous chamber depth (M5), optical disc length (M6) and optic nerve diameter (M7). Colored Doppler identified the external ophthalmic article and pulsed Doppler assessed its flow, and the following were measured: systolic peak velocity (VPS), final diastolic velocity (VDF), resistivity index (IR) and pulse index (IP). No statistical difference was observed for the biometric values of the eye segments between the right and left eyes (p>0.05). The vitreous chamber depth (M5) was shown to be the biometric variable with greatest bilateral symmetry, varying from 0.79 to 0.87cm and 0.78 to 0.86cm for the right and left eye, respectively. The ophthalmic artery was visualized over the optic nerve towards the eyeball, with flow stained red. There was no significant statistical difference between the Doppler velocimetric values for the ophthalmic artery between the right and left eye of the animals assessed (p>0.05). The mean resistivity index (RI) showed average values equal to 0.63±0.03, bilaterally. The mean base velocity was 17.50cm/s and 18.18cm/s at the systolic peak and 6.21cm/s and 6.68cm/s at the end of the diastole, for the right and left eyes respectively. The anatomic, biometric and hemodynamic characterization using the ultrasound B-scan and the Doppler modalities permitted the ultrasonographic and Doppler velocimetric assessment of the eyeball components in dogs of different breeds, and it can be used in ophthalmic clinical routine to identify eye pathologies.

The black-rumped agouti ( Dasyprocta prymnolopha , Wagler 1831) is currently under intense ecologic pressure, which has resulted in its disappearance from some regions of Brazil. Echocardiography is widely used in veterinary medicine but it is not yet part of the clinical routine for wild animals. The objective of the present study was to assess the applicability of the echocardiographic exam in nonanesthetized agouti and to establish normal reference values for echocardiographic measurements in bidimensional mode (2D), M-mode, and Doppler for this species, and a lead II electrocardiogram was simultaneously recorded. Twenty agouti were used in this study. All the echocardiographic measurements were positively correlated with weight (P < 0.05), and there were no significant differences between sexes (P > 0.05). Blood flow velocities in the pulmonary and aortic artery ranged from 67.32-71.28 cm/sec and 79.22-101.84 cm/sec, respectively. The isovolumic relaxation time was assessed in all the animals and ranged from 38.5 to 56.6 ms. The maximum value for the nonfused E and A waves and the Et and At waves was 158 beats/min for both. The results obtained for the morphologic and heart hemodynamic measurements can guide future studies and help in the clinical management of these animals in captivity.
